What is the driver questionnaire?
We recognize that a policyholder's household information can become outdated due to changes in driving activities, drivers, vehicle usage, and/or employment or residence. The driver questionnaire is designed to help us keep up with your changing insurance needs. To accomplish this, we are using the assistance of an experienced insurance information service provider - URI Information Services.
Policyholders will be mailed a questionnaire from URI Information Services requesting information on how they use their vehicles and information about drivers in the household. The questionnaire is easy to understand and will only take a short period of time to complete.
If you are a Westfield policyholder and receive this questionnaire, please complete and return in the postage-paid envelope within the requested timeframe. If you have any questions, please contact your agent.
